個案管理是一種以個案為中心,由各類醫療人員參與照護,達到預期的照護品質及控制醫療費用的照護系統。為達到照護之連續性及即時性,個案管理系統資訊化為不可或缺之一環。建立個案管理資料庫有助於提升資料管理效率、減少資料的重複性、提高資料安全性與保密性等,並且能夠提供自動提示的功能及方便資料統計分析等。資料庫建置前必須考量資料庫要涵蓋的功能,充分與資訊人員溝通合作,建置完成後可運用於個案管理過程之各個階段,協助個案篩選、連結個案照護之相關資訊、作為個案管理照護紀錄、各項指標監測、保險申報參考、成本分析及研究等,結合強大的資訊自動化功能不但能確保資料正確性與即時性,更能提高個案管理之服務品質與效率。 |
英文摘要 |
Case management is a case-centered care model that leverages multi-disciplinary teamwork to provide quality and cost-effective patient care. To ensure care continuity and timeliness, it is necessary to automate case management systems. A case management database is essential to system efficiency; helping avoid double data entries and ensuring data confidentiality and system security. The database can also support data analysis and programming designed to generate useful statistics and automatic reminders. Identifying critical database functions is the first step in database development. Once a database has been established, it can be applied to functions throughout the case management process, including case screening, care related data retrieval, patient record storage, quality monitoring, insurance reimbursement and research. An effective information automation system will ensure the accuracy and timeliness of information as well as enhance case management quality and efficiency. |
