目的：小柴胡湯在臨床上的使用相當常見，且也包含有眾多的適應證，無論台灣或是日中也都有不少相關的研究被報導，然而對於小柴胡湯的正、反作用則一直存在有莫衷一是的看法，因此本文即係以健保資料庫、回顧性文章和病例報告等來探討小柴胡湯對於肝臟的影響。方法：搜索西元2000至2018年間PubMed、Cochrane等英文資料庫，英文關鍵字包括Xiao-chai-hu-tang、sho-sai-ko-to、herbal medicine、hepatitis、liver injury等，進一步加以統整與討論分析。結果：綜合上述資料庫所蒐錄到的文獻，統合並扣除不符需要的研究之後，一共搜索出11篇證據等級較高之文章，並分析其正反面論點。結論：小柴胡湯對肝臟有保護作用，但也會在特定情況下產生不良反應，未來期許能訂定出小柴胡湯之安全劑量、致毒劑量等，讓臨床醫師能有清楚的處方準則。

Purpose: The clinical use of Xiao Chai Hu Tang is quite common and contains many indications. Many related studies have been reported in Taiwan or Japan and China. However, the positive and negative effects of Xiao Chai Hu Tang have been reported. There are inconsistent opinions, so this article is to explore the effect of Xiao Chai Hu Tang on the liver with a health insurance database, retrospective articles and case reports. Method: Search English databases such as PubMed, Cochrane from 2000 to 2018, English keywords include Xiao-chai-hu-tang, sho-sai-ko-to, herbal medicine, hepatitis, liver injury, etc. Discussion and analysis. Results: Based on the literatures collected in the above database, after deducting the research that does not meet the requirements, a total of 11 articles with higher levels of evidence were searched out, and their positive and negative arguments were analyzed. Conclusion: Xiao Chai Hu Tang has a protective effect on the liver, but it can also cause adverse reactions in specific circumstances. In the future, it is possible to determine the safe dose and toxic dose of Xiao Chai Hu Tang, so that clinicians can have clear prescription guidelines.
