穴位按壓對癌性疲憊之成效：系統性文獻回顧

目的：穴位按壓是相當方便、安全且花費少，又容易被病人接受的疲憊緩解方法，然而，目前國內、外從未有系統性文獻回顧聚焦在穴位按壓對緩解癌因性疲憊的效應，進而影響臨床應用。因此，本研究透過系統性文獻回顧，探討穴位按壓對於緩解癌因性疲憊的效應。方法：從PubMed、Cochrane Library及Airiti Library華藝進行文獻搜索，關鍵字為「Neoplasms」、「Acupressure」、「Complementary medicine」、「Fatigue」、「疲憊」、「癌症」、及「穴位按壓」；納入2004-2019年中文及英文的隨機控制試驗。以Newell 10大內在效度指標進行文獻評讀。結果：共計6篇文獻符合納入條件。共573位治療中或完成治療的癌症病人，接受不同劑量的穴位按壓。結果發現5篇文獻為高品質研究，達中～高度效應值，僅1篇研究為中度品質研究，僅低～中度效應值。結論：穴位按壓可有效改善癌因性疲憊，建議按壓合谷（LI4）、三陰交（SP6）、及足三里（ST36），每天每個穴位按壓至少1分鐘，並鼓勵病人持續進行按壓。未來期望能訂定緩解癌因性疲憊的穴位按壓指引，提供醫護人員臨床操作或指導病人及家屬時的參考。

Purpose: Acupressure has been applied to the relief of cancer-related fatigue (CRF), which is quite convenient, safe, inexpensive, and easy to be accepted by patients. However, there was no systematic review has purely focus on the effect of acupressure on the relief of CRF so far. Therefore, the aim of this systematic review is to determine the efficacy of acupressure on CRF. Method: A systematic search of PubMed, Cochrane Library and Airiti Library was conducted. keywords used in the search included ＂neoplasms＂, ＂Acupressure＂, ＂Complementary medicine＂ and ＂fatigue＂. Only randomized controlled trials (RCT) from the years 2004-2019 were included. Studies were graded and appraised by the Newell's 10 internal validity indicators. Results: Six studies were found and included in the analysis. A total of 573 cancer patients under or complete treatment were received different levels of acupressure. Five studies were good-quality research, which had moderate to high effect size. Only one study was fair-quality research, which had low to moderate effect size. Conclusions: Acupressure can improve patients' fatigue. To relieve CRF of cancer patients, Hegu (LI4), Sanyinjiao (SP6), and Zusanli (ST36) can be pressured daily for 1 minutes, while encouraging patients to continue to press. In the future, it is possible to develop an acupressure guideline on reducing CRF, so that the healthcare personnel can have a reference to use for clinical care or teach cancer patients and their caregivers.
