病人辨識手環模糊率及其影響因素

本研究旨在調查某醫學中心住院病人辨識手環模糊率並分析影響辨識手環以下簡稱辨識環是否模糊之使用者與照顧者因素。調查方式採一日普查法(one day survey)；樣本爲該醫學中心民國93年9月10日當日之所有住院病人。資料之蒐集係由護理人員（共119人）攜帶結構式觀察紀錄表，至病人床旁觀察當時的辨識環上七項病人資料（包括：姓名、病房、床號、出生年月日、病歷號、保險身份、科別及性別）清晰程度並填寫，共觀察有效樣本843人。結果顯示病人辨識環模糊率高達20%，造成辨識環模糊的主要因素爲使用標籤的天數及有否使用保護膜。故該醫學中心已於93年11月8日公告並規定實施：住院第一天於病人辨識環上標籤加貼透明膠帶保護膜的措施，以確保住院期間病人辨識環的清晰，進而維護病人安全。

The purposes of this study were to understand the hazy rate of patient bracelet identification and its related factors in terms of the aspects of patients and their caregivers. The one-day survey (September 10,2004) was implemented in a medical center; observers went to the inpatients' bedside to collect data with a structured list. The results revealed that the hazy rate reached 20% and the main factors that caused haziness were days of wearing and whether a protective sleeve was put on the bracelet. To prevent the haziness of bracelet identification, the medical center has adapted the policy of putting sleeves on bracelet for every inpatient.
