精神病人重覆入院常造成家屬負擔及醫療成本浪費,而預防疾病復發的重要方法就是定期回診,故本專案旨在降低精神科急性病房病人出院未回診率。根據本院精神科出院病人門診追蹤月報表2005年12月至2006年2月間,精神科急性病房病人出院後7-30日內未回診率為24.42%。經分析原因後,實施製作衛教單張、修訂出院流程及專人電話諮詢並提醒回診之解決方法後,未回診率由24.42%降至6.67%,因協助回診而提昇醫院服務品質。期望藉此專案嘉惠更多出院病人,能及時回診,遵從醫囑服藥,以穩定症狀,提昇社區適應。 |
英文摘要 |
Regular outpatient treatment is so important. Repeated hospitalization of mentally ill patients is a great burden to the family members both emotionally and financially. This project aimed at reducing the outpatients' follow-up noncompliance rate. The overall outpatient noncompliance rate was 24.42% from Dec 2005 to Feb 2006. This project provided enhanced health education leaflets, amendment of discharge process and reminding of appointment date via telephone by hospital staff. The project significantly improved the noncompliance rate from 24.42%to 6.67%. This project effectively enhanced the quality of hospital services, improved the outpatient follow-up compliance rate, stabilized patients' symptoms and helped the patients go back to the community. |
