手術壓力性損傷乃因病人在手術中制動與擺位等因素引起的皮膚與組織缺損，是重要的照護品質指標。本單位自2016年起壓力性損傷發生率持續上升，高於過去三年的平均值，專案小組分析原因為無手術室專屬壓力性損傷風險評估表、現行標準作業流程不敷使用、無預防性敷料、缺乏教育訓練與稽核制度等。經建立壓力性損傷評估表、修訂預防手術壓力性損傷標準作業流程、使用減壓預防性敷料、教育訓練和建立稽核制度等解決方案，手術壓力性損傷由改善前0.30%降為改善後0.23%。藉由照護模組概念，建立實證照護標準作業，強化照護知能及技能，進行品質監控，有助於提升醫療照護品質。 |
| 英文摘要 |
Intraoperative pressure injuries are characterized by damage to skin and tissue caused by immobility, positioning, and continuous pressure during surgery. Their incidence indicates perioperative care quality. Since 2016, the incidence of pressure injury in our unit has increased significantly. We considered the following problems as the major causes: (1) inadequate current standard operating procedures, (2) lack of surgery-specific pressure injury screening tools, (3) lack of prophylactic dressing, and (4) shortage of educational and auditing systems. Therefore, we established an intraoperative pressure injury risk assessment form, training program, and audit system. We also revised the standard operation procedures and included prophylactic dressings in the skin care bundle. After implementation, the incidence rate of intraoperative pressure injuries decreased from 0.30% to 0.23%, suggesting an improvement in perioperative care quality. |
