本文描述一位73歲肝硬化合併腹水及肝性腦病變的女性病人，面對身體、心理及社會層面改變之照護經驗。照護期間為2020年6月19日至7月30日，筆者收集身、心、靈與社會層面的資料，並以Gordon十一項功能性健康型態進行整體性評估，歸納健康問題為體液容積過多、急性混亂、無效性因應能力。在護理過程中，透過擬定適當的護理計畫，提供具個別性、有效性之實證護理措施，及聯繫跨專業醫療團隊專家介入的協助，以改善病人的健康問題，進而提升其出院後之生活品質，達到全人、全隊、全程、全家及全社區之照護。希望藉由此護理經驗分享，提供未來護理人員照護肝硬化病人之參考。

This paper describes a nurse's experience caring for a 73-year-old female patient with liver cirrhosis, ascites, and hepatic encephalopathy, in the face of physical, psychological, and social changes, from June 19 to July 30, 2020. Data on the patient's physical, mental, and social statuses were collected and a holistic assessment was performed based on Gordon's 11 Functional Health Patterns. The main health problems were excessive body fluid volume, acute confusion, and insufficient response capacity. Effective nursing strategies, individualized care, and contact with professional medical teams can improve the patient's health and quality of life after discharge. To achieve the care of the whole person, the whole team, the whole process, the whole family and the whole community. This case study can provide a reference for nurses caring for patients with liver cirrhosis.
