「純字聾」(Pure Word Deafness)早在1877年就有學者提出報告。這類患者具有很好的口語表達、閱讀和書寫能力,但是對口語的了解力却有嚴重的障碍。「純字聾」並非聽力受損,而是因腦傷後所造成的聽覺不識(auditory agnosia)。近幾年來,許多學者致力於造成此障碍的腦部機轉的研究,特別是「聽覺傳導過程」(auditory processing)的探討。本文報告一位純字聾的病例,並以「聽覺傳導過程」來研究其聽覺了解力障碍的特性。結果顯示此病例在「語言譯碼過程」(linguistic decoding)上有明顯障碍。因這類病例,在國內十分少見,故特別提出報告。 |
英文摘要 |
Pure word deafness is a rare syndrome first described by Kussmaul in 1877. These patients are unable to comprehend spoken words with other function intact, such as speech production, reading and writing abilities. Clinically, the patient with pure word deafness is supposed to be free of any aphasic difficulties, although many of the reported cases are marked by elements of Wernicke's aphasia. Some authors have preferred the term ”verbal auditory agnosia” or ”auditory agnosia for linguistic material”, but the term ”pure word deafness” has retained wide clinical acceptance.Recently, investigators have attempted to characterize the nature of this deficit, and have emphasized the role of auditory processing. We have recently had the opportunity to study a patient with the syndrome of pure word deafness associated with milder agraphia. On the basis of a detailed neuroanatomical and neurolinguistic studies, we found a disordered pattern of semantic, pragmatic, and some syntactic decoding that could be attributed to the patient's disorder in auditory processing. |
