高移位股骨頸骨折是一種少見的病例，病人常伴隨有神經肌肉系統的障礙。我們報告一例罕見的病例，在三週的骨骼牽引後，以一階段全人工關節置換術來加以治療，術後再加以皮膚牽引兩週，病人術後沒有任何合併症發生。在此我們提出病例，並整理有關文獻，發現高移位的股骨頸通常可見於陳舊性的先天性髖節脫位或外傷性關節脫臼，以一階段手術通常需要縮短股骨幹，否則則需二階段手術治療。因此我們認為對於高移位的股骨頸骨折以一階段全人工關節置換術治療，術前的骨骼牽引及手術中神經張力的探查，是避免神經傷害的最好方法。

We report an unusual case of neglected high-displaced femoral neck fracture. One-stage total hip arthroplasty was performed afte 3 weeks of distal femoral skeletal traction. Postoperatively, the operative limb was immobilized with Buck's traction for 2 weeks. The patient was encourged to start quadriceps exercise on the second post-operative day and during the traction peroid. Ambulation with crutches and partial weight-bearing started after removal of the traction. At the 2 year follow-up, the patient was doing well without any complications.
