表現CD5標誌之B淋巴球,其所產生之免疫球蛋白,常可與自已成份結合,因此被認為在自體免疫疾病中可能扮演重要角色。以住,許多在洋人所做的研究都顯示此類B淋巴球在類風濕性關節炎病人呈現明顯的增加。但由最近一些報告,發現此類淋巴球之多寡似乎由遺傳基基所控制。我們乃以29位中國病人為究對象,測其血中此類CD5陽性B淋巴球所佔之比例。結果,我們發現類風濕性關節炎病人之CD5陽性B淋巴球並沒有較正常人多。同時,病人之此類B細胞之多寡也與疾病嚴重程度,或類風濕因子濃度高低沒有密切關聯。在六位病人一年之追蹤下,也顯示CD5陽性B細胞在個人都維持頗為隱定之比例。因此,我們結論認為在國人之類風濕性關節炎疾病,CD5陽性B淋巴球並不扮演重要角色。此類B細胞之多寡受個人遺傳基因所影響。(慈濟醫學1994;6: 145-151) |

Most studies conducted in Caucasians showed increased circulating CD5 positive B lymphocytes in rheumatoid arthritis (RA) patients. Since CD5+B cells expression appeared to be under genetic control, we investigated its distribution in twenty-nine Taiwanese patients with RA. Cir-culating CD+5 B cell levels were analyzed by flow cytometry. There was no elevation of CD5+B cells in RA patients compared with healthy subjects. The population of CD5+B cells correlated neither with titers of serum rheumatoid factors (RFs) nor with disease activities in RA patients. The proportion of CD5+B cells in the peripheral blood remained relatively constant after one year follow-up studies in six patients. The similar pattern of epitope density of CD5 antigens was also observed in the same individual. We thus concluded that circulating CD5+B lymphocytes played little role in the pathogenesis of RA. Levels of CD5+B cells reflected more individual genetic background rather than susceptible marker for RA in Taiwanese. (Tzu-Chi Med J 1994; 6: 145-151) |
