Preoperative Ultrasound-Guided Wire-Localization of Nonpalpable Solid Breast Lesions

自1985年3月至1989年2月，我們嘗試以各種不同之金屬絲(wires)作乳房小病灶之術前定位，包括一種自製之帶鉤銅絲(hooked copper wire)及二種廠製帶鉤鋼絲(stainless steel hookwire)，共於25位患者進行27次定位。患者皆係XRMG無異常或為明顯緻密乳房(dense breast)而接受USMG檢查，因有不易評估之固質結節而後決定做超音波導引之術前定位。超音波檢查(US)係以10MHz扇形探頭全面掃視二邊乳房，於偵得懷疑之病灶時將探頭置於病灶正上方，並於離探頭中心1-3cm處之合適進針位置在皮膚上作上記號，在皮膚消毒及局部麻醉後，以長5-7cm之針（內置帶鉤之銅絲或鋼絲）於記號處刺入皮下，探頭則置於病灶表面導引進針方向（通常針與探頭夾30-75度角），針尖在US下常呈一強反射之亮點，於實時間下(real-time)觀察，直到針尖穿過病灶中心，抵達病灶側邊綠，或穿出病灶後緣1cm後，即固定鋼（銅）絲，再移出其外之針。手術中由外科專家記錄鋼（銅）絲之位置及固定之情況，如為牢固，則定為“好”，如不慎以大力而掉出，則為“可”，如只於術中動作下即掉落則為“差”。最後，此等小病灶之病理亦加以分類記載。結果顯示：自製銅絲固定效果不好（好：2/7），廠製鋼絲則甚佳（好：17/20），當固著良好時，外科醫師對定位之效果皆表滿意。89% (17/19)鋼絲精確地穿過病灶，有二例因為病灶太小(0.4及0.6cm)，鋼絲則落於病灶旁0.5cm之距離以內，但對手術仍有助益前述27個結節之病理結果仍以良性居多，而19%為惡性結節(5/27)。無一患者因定位而有併發症。所有第二次XRMG仍皆無法見到USMG所見之病灶。我們相信超音波導引鋼絲定位為一有效而安全之方法，對引導手術摘除臨床上無法觸得之結節頗為有用。

Twenty-seven wire-localization procedures were performed under ultrasound-guidance in 25 patients with nonpalpable solid nodules of the breasts. All the patients referred for these procedures had had a negative X-ray mammographic examination (usually with radiographically dense breasts) but a positive ultrasonic mammography (USMG). The wire used included homemade hooked copper wires and stainless steel hookwires. Fixation was good in 19 procedures, fair in 4 procedures and poor in the other 4. In the 19 procedures with good fixation, 17 (89%) were of accurate localization, while the other 2 had the wires localized within 0.5-cm distance to the lesions. The pathologic reports of these small solid nodules were 22 benign and 5 (19%) malignant lesions. No complication was encountered after the localization procedures. US-guided localization with stainless steel hookwire is a easy, quick and accurate procedure to aid the surgeon for a precise removal of a nonpalpable solid breast nodule.
