張眼困難症乃支配眼瞼張開的肌肉、神經功能仍正常，卻無法隨意起始張閉眼睛之疾病。1927年Schilder首次提出病例報告。本篇報告一例男性病患因腦血管疾病造成左眼單側性張眼困難症，向其病灶位置及相關檢查，討論其臨床變化與可能之致病機轉，特提出本報告。

A victim of bilateral thalamic infarction, 60 years old, male, was submitted to our oph. section with the chief complaint of left small lid fissure was noted post stroke four months ago. The outstanding highlights in this case was involuntary levator inhibition with reflex blepharospasm of left eyelid. Neurological exams showed cranial nerve No. 2. 3. 5. 7 were defectively involved; Laboratory data including CTs can-Brain, V.E.P, E.E.G. revealed abnormal findings. It was thoroughly described in the article. We reported this case base on the rare condition.
