Application of Captopril Renography in a Child with Renovascular Hypertension

Captopril腎臟造影應用於診斷孩童腎血管性高血壓的報告不多。一位13歲女孩因爲最近幾個月來血壓升高而來求診，理學檢查及腎臟超音波檢查均無異常發現，但是血漿中腎素濃度升高，Captopril腎臟造影顯示右側腎臟功能在服用Captopril之後明顯下降，判斷爲高度可能性羅患腎血管性高血壓，病患隨後接受腎血管攝影檢查，證實右側腎動脈狹窄，經過氣球血管成形術之後，血壓及血漿中腎素濃度均恢復正常。我們認爲Captopril腎臟造影對於孩童腎血管性高血壓的診斷與治療很有幫助。

The applications of captopril renography in the pediatric patients with renovascular hypertension were reported relatively less common. A 13-year-old girl was found to have elevated blood pressure for several months. Physical examination and renal ultrasonography were unremarkable. Plasma renin concentration was elevated. Captopril renography revealed decreased renal function of right kidney after administration of captopril, suggestive of high probability of renovascular hypertension. Renal angiography demonstrated right renal artery stenosis. After balloon angioplasty, blood pressure and plasma renin concentration returned to normal ranges soon. We suggest that captopril renography is helpful for the diagnosis and management of children with renovascular hypertension.
