尿液常規檢驗包含尿化學檢驗及尿沉渣檢驗是健康檢查常見的篩檢項目,也是臨床醫師對於結石、尿路感染、腎臟發炎等疾病診斷及預後治療效果判斷的重要工具之一。但傳統尿沉渣人工鏡檢法需耗費時間與人力且有諸多干擾因素而影響報告之準確性。近年來自動尿液分析儀已逐漸應用於臨床實驗室,本篇研究分析iQ200尿沉渣自動分析儀與尿沉渣人工鏡檢法之結果。取病患隨機尿液以iQ200尿沉渣自動分析儀分析尿液細胞粒子,再將尿液離心後以人工鏡檢判讀尿沉渣之結果,分析兩者之相關性。結果iQ200尿沉渣自動分析儀與尿沉渣人工鏡檢法,兩者尿液細胞粒子結果之一致性皆大於90%。iQ200藉由圖像呈現尿液細胞粒子,可將尿沉渣結果標準化,但因其原理是以尺寸大小、形狀、對比度、紋理等方式分析,所以仍有干擾現象,需以人工編輯圖像才能使結果更精確。iQ200有別於其它尿液自動分析儀必須以人工鏡檢法來排除干擾原因。將iQ200拍攝之圖像以人工審查編輯後即可發報告,可降低人工鏡檢率,進而簡化檢驗流程、縮短檢查作業時間,提升醫療品質。

Urine routine test is one of the most common laboratory tests in Health Check. It may be a powerful tool in clinical diagnosis of various diseases such as renal disease and urinary track infection. However, the conventional manual microscopic examination of urine sediment is a time consuming and labor intensive procedure. Besides, the result may be affected by many interfering factors. In recent years, automated urine sediment analyzers have been increasingly used in clinical laboratories. In this study, we compared the result of iQ200 automated urinalysis analyzer with manual microscopic examination. Random urine specimens were collected and analyzed via iQ200 automated urinalysis analyzer, and then centrifuged urine specimens were analyzed by manual microscopic examination. The correlation of the results by two different examination procedures was evaluated, and the agreement of the both was greater than 90%. The iQ200 automated urinalysis analyzer standardizes the results of urine sediment examination by categorizing the images of urinary cells and particles. But because the principle is based on the size, shape, contrast, and texture, it may be interfered by other ways. Manual editing is required to elevating the accuracy. Unlike the other automated urine sediment analyzers require manual microscopic examination to exclude interference. Manual editing of stored image captured by iQ200 automated urinalysis analyzer could eliminated interference, and could reduce the requirement of manual microscopic examination. The application of iQ200 effectively lowers the manual microscopic review rate. Furthermore, it simplifies the examination procedure, shortens the turn around time, and improves the quality of care.
