Spontaneous esophageal perforation, also known as Boerhaave syndrome or effort rupture of the esophagus, is most lethal if treatment is delayed. Different perforation regions contribute to a variety of clinical symptoms and signs. The rarity and variability of the clinical signs of esophageal perforation may lead to a delay in diagnosis. According to a previous literature review, the mortality rate was significantly lower when a primary repair was performed within 24 hours after perforation. Here, we reported on the emergency surgical repair of a 40-year-old man diagnosed with Boerhaave syndrome with right epigastric pain and coffee-ground vomitus after meals.
