運動猝死一般指的是運動中或運動後24小時內,因爲運動造成的死亡。這包括了因爲腦部血管破裂造成的腦溢血、腦部血管栓塞造成的腦部缺氧、動脈剝離或破裂造成的血壓急速下降及心律不整引發心臟停止跳動(cardiac arrest)等急性因素所造成的死亡。由於心臟細胞動作電位的自發性(spontaneous)及同步性(synchronized),使心臟潛藏了可能發生問題的危機。因爲一個不正常心臟細胞的興奮,就可能造成整個心臟不正常的收縮。遺傳造成的膜孔蛋白質變異,可能影響了心肌細胞動作電位時細胞離子的正常進出、或心臟細胞內鈣離子的儲存及釋放,嚴重時可引發心律不整或猝死。另外,病理性的肥胖或高血壓,造成心臟局部細胞環境的改變,這造成細胞膜上膜孔再造(ion channel remodeling),改變了正常心臟細胞復極電位波形(repolarization waveform)或造成心臟細胞復極電位及不反應性的分散(dispersion of repolarization and refractoriness)。以上因素可能造成少數心臟細胞動作電位的不同步,進而產生再進入電路(reentrant pathway)、異位性心跳(ectopic beats)及心律不整(arrhythmia),嚴重時甚至會造成猝死,這就是所謂的突然心律不整死亡症候群(sudden arrhythmic death syndrome)。由於心臟運作機轉的複雜性及它對生命的重要性,有心律不整或心電圖異常的人,運動前最好聽取醫生的意見或服用藥物。
