動機:2018年某醫院大火造成多人傷亡,突顯醫療院所消防應變能力之重要性,特別是加護病房,病人常需使用多項維生儀器設備,當火災時因行動不便且逃生能力有限,造成嚴重傷亡。統計2018年10月15日至2018年10月20日發現團隊人員對於消防應變認知正確率71.1%、消防應變技能正確率64.3%,其缺失有:一、設備面:氣體開關、排煙閘門及防煙垂壁標示不清;二、政策面:單位的消防防護計畫內容不完善、單位的教育訓練不完善、無實際消防設備操作演練;三、人員面:不熟悉各消防編組任務及消防設備位置、不瞭解防火區域之劃分。目的:提升內科加護病房人員消防應變能力。解決方法:藉由修訂單位消防防護計劃、增設消防應變演練計畫書、製作防火區域平面圖、錄製消防設備操作影片、並舉辦消防暨實務演練教育訓練。結果:團隊人員對消防應變認知正確率由71.1%提升至93.5%、消防緊急應變技能正確率由64.3%提升至達99.3%,有效提升人員消防應變能力。結論:運用多元化課室教學、六種案例情境演練、消防設備操作影片及QR code等策略,提升人員應變能力,希冀可供同仁參考。

英文摘要

Background: Noted major injuries with hospital fire in 2018, shows how important to promote personnel response when fire happen. Especially in intensive care units (ICU), patients used multiple life-sustaining instruments and equipments, which caused heavy casualties due to inconvenience and limited escape capacity during the fire. According to statistics from October 15, 2018 to October 20, 2018, ICU members were found. The correct rate of the knowledge of fire emergency response is only 71.1%, skills of fire emergency response is only 64.3%. The defects includes, 1. Device-wise: Gas switch, smoke valve and smoke-prevention door are indistincly marked; 2. Policy-wise: incomplete ICU fire prevention plan and training didn’t actual equipment operation training; 3. Personnel-wise: The staff is not familiar with each fire formation task, the location of fire-control equipment, and fireproof compartment. Purpose: To improve ICU members of fire emergency response. Resolutions: by establishing a fire prevention plan, adding fire drill plan, generating floor plans for fire-resistant areas, recording fire equipment operation video, and practice method, etc. Results: After the implementation of diversified classroom teaching, fidelity simulations, fire equipment operation video and QR code, the knowledge of fire emergency response showed improvement from 71.1% to 93.5%, skills of fire emergency response showed improvement from 64.3% to 99.3%. The purpose of this project was achieved.
