本研究目的旨在探討影響慢性阻塞肺疾病病患生活品質相關因素。研究採橫斷式相關性研究設計,以立意取樣,針對中部三家教學醫院門診就診之慢性阻塞性肺部疾病病患,以結構性問卷進行資料收集。研究問卷包含台灣版世界衛生組織生活品質問卷簡明版(WHOQOL-SF)、聖喬治呼吸問卷(SGRQ)、情緒狀態量表(POMS-SF)和個人基本資料。共收案124位病人進行資料分析。研究結果發現影響WHOQOL-SF與SGRQ得分的因素各有不同,與其問卷所建構的內容相符。雖然FEV1能預測SGRQ總分和活動得分,但可以預測WHOQOL-SF 心理範疇和總分及SGRQ衝擊得分的肺功能指標卻為FEV1/FVC比值。即便如此,兩份量表得分分別與POMS-SF各次量表得分呈現弱到中度的相關,而且在控制其它顯著變項影響後,混亂困惑、精神活力、疲憊懶散、憤怒敵意、憂鬱焦慮等情緒分別可以預測WHOQOL-SF或SGRQ的各項得分。研究結果可做為進一步探討生活品質相關議題,或做為設計自我照顧策略,改善病人生活品質的依據。 |
英文摘要 |
The purpose of this study is to explore the factors affecting quality of life of patients with chronic obstructive pulmonary disease. The research is a cross-sectional, correlation design. A purposeful sample was drawn from the OPD clinics of three teaching hospitals in central Taiwan. Four structured questionnaires were used for data collection including World Health Organization Quality of Life-Short Form (WHOQOL-SF), St. George's Respiratory Questionnaire (SGRQ), Profile of Mood States- Short Form (POMS-SF), and basic demographic data sheet. A total of 124 responses were analyzed in the study. The results of the study indicate that factors influencing the diverse dimensions of the quality of life are comparable to the theoretical constructs of the tools. Although FEV1 predicts the total scores and activity of the SGRQ, it is the ratio of FEV1/FVC predicts the psychological dimension and the total score of the WHOQOL-SF as well as the impact of the SGRQ. There are weak to modest correlations between the scores of WHOQOL-SF/SGRQ and the scores of POMS-SF. The analysis of multiple regressions indicates confusion-bewilderment, vigor-activity, fatigue-inertia, and anger-hostility are strong predictors for the quality of life of COPD patients. The findings of the study can be used as a basis for further study as well as for designing effective strategies for improvement of patient health related quality of life. |
