產後護理之家產婦憂鬱情緒之前驅研究

本研究是探討產後婦女於產後護理之家期間憂鬱程度與變化。採立意取樣，參與者為50位來自東部某產後護理之家的產婦。並以中文版愛丁堡產後憂鬱量表及半結構式訪談進行資料收集。研究顯示，婦女入住產後護理之家2週，憂鬱程度較剛入住時為低（平均值7.36 vs. 5.84; t=4.42; p＜.01）；剛入住時有28.0%的婦女，愛丁堡產後憂鬱量表分數高於9分，達產後憂鬱程度，入住2週後產後憂鬱人數下降至12.0%，並達統計顯著差異(x^2=4.42; p=.02)。入住產後護理之家，母兒能隨時獲得專業照護並學習育兒技巧，及能有足夠的時間與空間獲得充分的休息，可能是正向影響婦女產後情緒變化之因素。本研究結果能提供健康照護人員了解婦女產後情緒變化及產後照護的需求。並建議產後照護單位與機構能將產後憂鬱篩檢納入常規的照護中，以關注並提昇產婦心理的健康。

This study aims to evaluate the status and changes of depression in postpartum women in post-partum nursing center. Using purposive sampling, the total sample size was 50 women in the post-partum nursing center in eastern Taiwan. The Chinese version of the Edinburgh Postnatal Depression Scale (EPDS) and semi-structured interviews were utilized to collect the data. The results showed that the difference in mean scores of EPDS-1 (EPDS score at admission day) and EPDS-2 (EPDS score after two week) attained statistical significance (mean 7.36 vs. 5.84; t=4.42; p＜.01). It was found that 28.0% of women fell into the postpartum depression category (EPDS score above 9) at EDPS-1. At EPDS-2, the rate fell to 12.0% with statistical significance (X^2=4.42; p=.02). When women lived in a post-partum nursing center, mother and child received professional care, mothers learned parenting skills, and had time and space for adequate rest, which maybe positively affect women's mood changes. The results of this study may provide healthcare workers with the capacity to recognize mental status changes in postpartum women. As such, we suggest that maternal care unit and institutions' services conduct screening for postpartum depression routinely to improve maternal mental health.
