This is a special communication contributed by Dr. T. Takashina at the invitation of the Editorial Advisor, Professor Chiu-Yin Kwan of China Medical University as he was invited to attend the 41st Congress of the Japanese Society of Medical Education held in Osaka and organized by the Medical School of Kinki University.
