哈佛醫學院作為世界醫學教育界的標竿,其醫學教育改革動向歷來備受各界矚目。近二十年來,台灣大學醫學院與哈佛醫學院的交換學生計畫不僅開拓學生的視野,也為台灣醫學教育界帶來許多新知與改變。筆者於2010年春季前往哈佛醫學院教學醫院交換見習,期間除了積極參與、觀察其臨床教育與醫療環境外,亦投入相當時間與哈佛師生進行深度訪談,本文將根據此趟交換見習收穫,就哈佛醫學院教育改革現況進行介紹。本文第一部分將介紹哈佛醫學院的教育理念、入學方式與課程設計。其次,將分三面向-專業素養、連貫式見習課程與課外進修資源,介紹哈佛醫學院教育改革現況,希冀對台灣醫學教育界帶來一些啟發。 |

Curriculum reforms at Harvard Medical School (HMS) have long been regarded as paradigms for medical schools around the world. The New Pathway (NP) M.D. program, an audacious attempt to reshape how medicine is taught at HMS, is now in its third decade. Despite its tremendous success, the NP program continues to evolve to meet the needs of students and society. Over the years, an exchange clerkship program between the National Taiwan University School of Medicine (NTUSM) and HMS has provided students with opportunities to learn about the two very different systems and has fostered significant changes in medical education in Taiwan. As a participant in the exchange program in the spring of 2010, the first author drafted this article to summarize the latest education reforms at HMS after keen observation at various HMS-affiliated hospitals (Brigham and Women's Hospital, Massachusetts General Hospital, and Cambridge Hospital) and a series of in-depth interviews with HMS faculty members, physicians, and students.In the first section of the article, the authors introduced the mission of HMS and its admission process. Second, the curriculum of the NP program is outlined along with remarks on tutorial-based teaching. The authors then described the latest education reforms at HMS, including courses targeting professionalism, integrated longitudinal clerkships, and enrichment programs. In the last section of the article, the HMS reforms are assessed with respect to their applicability to Taiwan's medical environment. By briefly introducing the current education reforms at HMS. we hope this article will spark novel insights to the problems faced by medical educators in Taiwan. |
