Purpose: This article introduces a novel low-cost arthroscopic knot-tying trainer combined with an on-line video-based assessment and feedback system, and reports preliminary evidence to support its content and construct validity. Method: 36 senior medical students and 24 residents participated in an arthroscopic knot-tying training course. A step-by-step video tutorial was used to instruct trainees on how to tie an arthroscopic knot. After repeated practice and satisfactory demonstration of knot-tying skills, trainees completed, video recorded, and then uploaded the video of seven knot-tying tasks. Online feedback was provided by assessors based on the videos, which were also evaluated according to the scoring system. At the end of the course, trainees completed a survey questionnaire to rate the value of this training process based on a satisfaction scale. Results: The post-course satisfaction survey indicated that the majority of trainees were satisfied with this course. The mean score of the 7 tasks for the medical students was 6.7 (6.3-7.0) while for the residents it was 7.3 (7.0-7.6). Scores of the resident group were significantly higher than that of the medical students group for all tasks except for the use of the knot pusher (6.7±0.2, medical students; 6.5±0.2, residents. P=0.129). Conclusions: This novel knot-tying trainer and web-based training system can be a useful tool for basic arthroscopic knot-tying training. Through a combination of video-instruction, hands-on practice, video assessment and online feedback, these training programs can guide independent practice and expedite proficiency in skills acquisition for both medical students and residents.
