Comparisons of Performance on Postgraduate Year Entrance Examinations Between Medical Students from 6-Year and Post-Baccalaureate Medicine Programs in Taiwan

Introduction: Kaohsiung Medical University (KMU) is the only institution in Taiwan that has both a school of medicine (M) and a post-baccalaureate (PB) program. The performance of graduates from these two programs has been a subject of interest in medical education. This study aimed to compare the performance of M and PB graduates from KMU on the postgraduate year (PGY) residency entrance program at KMU-affiliated hospitals. Methods: This study included KMU graduates who applied for the PGY residency program affiliated with KMU Hospital from 2020 to 2022. Differences in basic information, rank in class (i.e, performance vis-à-vis one's peers), PGY interview scores, and PGY exam aggregate scores were analyzed to determine whether disparities in performance between M and PB graduates were present. The study also examined the differences in the percentage of M and PM students admitted within the enrollment quota, students who were final admittees, and students who opted for KMU. Results: Of the 422 KMU graduates who applied for the PGY residency program, 324 (76.8%) were M graduates and 98 (23.2%) were PB graduates. The proportion of M applicants surpassed that of PB applicants (X^2 = 17.55, p < 0.001). PB graduates had a significantly higher class rank (Precision-Recall (PR) value 65.6) than M graduates (PR value = 65.6 vs. 51.1; t = -4.30, p < 0.001). However, no significant differences in interview scores, PGY exam total scores, or admission rates between the two programs were found. Conclusions: Compared with M graduates, PB graduates had fewer applicants for the KMU-affiliated hospital PGY residency program but had superior class rankings. No significant differences were found in interview performance, total score, or admission rate between the two programs. These findings may serve as a valuable reference for future medical education reforms in Taiwan.
