影響精神分裂症急性住院患者藥費之因素

目的：本研究希望能在同時考慮“醫療資源有限”及確保“病人照護品質”的雙重前提下，透過藥費影響因素的探討，引發醫療人員對藥物適切性的關注，而能謹慎選擇合適之用藥。方法：依病歷記錄蒐集急性住院病房分屬五位精神科醫師之67位精神分裂症患者的基本資料、疾病史、及「調查當日藥費」等資料，以PANSS評估患者調查當日疾病嚴重度；再以卡方檢定、Kruskal-Wallis檢定、Mann-Whitney U檢定、雙變數相關分析等統計學方法進行各項影響「調查當日藥費」之因素分析。結果：發現「調查當日藥費」，於個別醫師之間差異達統計意義（p = 0.040），與患者發病年齡之間具有顯著性負相關（p = 0.029），與抗精神病藥物費用及情緒穩定劑費用之間具有顯著性相關（p = 0.001，p = 0.013）。結論：個別醫師之間的「調查當日藥費」具有顯著性差異；患者發病年齡、抗精神病藥物、及情緒穩定劑的使用，可能會影響精神分裂症急性住院患者藥費。
