目的:探究使用clozapine病人之白血球過多現象及此類病人的特質。方法:採用病歷回溯方式收集中部某醫學中心精神科使用clozapine病人的資料。使用卡方及獨立樣本T檢定分析產生與未產生此現象病人之差異,並進一步使用邏輯斯迴歸分析白血球增生的相關因子。結果:53.7%(41位中有22位)的病人有短暫或慢性的白血球增生,在產生白血球增生現象的這組病人其開始使用clozapine時的患病時間顯著的比沒有此現象的病人短(t=2.892, df=39, p=0.006);在基準點白血球數目也顯著地比沒有此現象的病人來的多(t=-2.195, df=39, p=0.034)。結論:在開始用clozapine前病人的疾病史越短可能和病人是否會出現白血球增生的現象有關。

英文摘要

Objective: To investigate leukocytosis in patients on clozapine treatment and to determine the characteristics of patients who developed leukocytosis. Methods: We did a retrospective chart review for patients who received clozapine in the psychiatric department of a medical center in central Taiwan. The differences between patients with and without leukocytosis were examined using Chi-square and t tests. The factors associated with leukocytosis were determined using binary logistic regression analyses. Results: In our study, we found that 53.7% of the patients (22 of 41) developed transient or chronic leukocytosis. Among patients who were taking clozapine, the duration of illness in those with leukocytosis was signifi cantly shorter than those without leukocytosis (t=2.892, df=39, p=0.006). Baseline WBC counts in the patients with leukocytosis were signifi cantly higher than the normal group (t=-2.195, df=39, p=0.034). Conclusion: This data leads us to infer that patients’ shorter duration of illness while taking clozapine may be associated with their development of leukocytosis.
