由於年齡、環境與行爲因素導致糖尿病人口不斷上升，糖尿病已成爲全球最重要的慢性疾病之一，可見糖尿病對國人的威脅已不容忽視，糖尿病非絕症，但卻需要長期控制，唯有讓病人學會自我照護，才能有效的控制疾病及預防惡化。本篇個案報告是護理一位糖尿病人者學習自我照顧的過程，在照護過程中筆者運用Gordon十一項健康功能型態評估，確立個案於住院期間之主要的護理問題。筆者於個案住院期間的護理重點爲給予個案正確的觀念及知識，使個案減輕因疾病引起的焦慮，及加強返家後的自我照顧能力。

Due to the factors of age, environment, and behavior, the diabetic patient population has been continuously increasing. Diabetes Mellitus is one of the major chronic diseases worldwide; therefore, its threat to our health should not be ignored. Diabetes Mellitus is not incurable, but needs long term control. The effective way to control disease with prevention of deterioration for the patient is to learn self-care. This article described the nursing process of a diabetic patient learning self-care. With Gordon's 11-item functional health pattern assessment, the patient's major nursing problems were identified. During the patient's hospitalization, the focus of nursing interventions were providing an accurate concept and knowledge of diabetes, reducing the patient's anxiety level secondary to disease, and reinforcing the patient's ability in self-care after discharge.
