運用芳香療法協助一位長者迴腸造口術後之護理經驗

本文描述一位71歲男性患者因腸阻塞行腸造口之術後護理經驗，筆者於2016年08月09日至08月22日經由直接照護、觀察、身體評估、會談及與家屬互動過程中，進行生理、心理、社會及靈性層面之整體性評估及分析，並確立患者有營養少於身體所需、急性疼痛及身體心像紊亂之健康問題。於照護期間發現家屬的芳香療法特長，故引發筆者想於加護單位中藉由實證搜尋芳香療法中不同精油功效搭配聞香、按摩、外部擦拭及室內淨化空氣的方式改善腸造口患者生理及心理的不適應，搭配個別性腸造口營養攝取、製作3D立體模型進行擬真照護衛教，並發揮傾聽及溝通技巧給予腸造口患者鼓勵及支持，最後得以引導患者正視術後身心像的改變並願意參與自我照護之內容，也改善患者及家屬對於加護病房冰冷的印象，盼以此結合芳香療法之護理經驗，可提供臨床護理人員照護此類患者之參考。

This article describes the nursing experience from caring a 71 -year-old postoperative male patient with ileostomy. The duration of nursing care was from the 9th to 22nd of August in 2016. Through direct nursing care, observation, physical assessment, communication, and interaction with family to assess and hence analyze the bio-psychosocial and physiological conditions. We were able to identify nursing problems such as nutrition imbalance that is less than body demand, acute pain and body image disturbance. During nursing care, patient's family aromatherapy expertise was discovered; it inspired the author to research different functions of essential oils by inhaling, massaging, wiping, and purifying air to reduce physical and mental discomfort. Additionally providing distinctive nutrition and educating the patient with a 3D simulated ileostomy. We successfully assist the patient in accepting the appearance change and willingness to participate in self-care through listening and communicative support. We also improved the traditional icy impression of intensive care unit from patients and their families. The objective of this article is to share this nursing experience that can hopefully serve as a reference and guidance to nursing care for this type of patients with aromatherapy companion for medical teams in the future.
