糖尿病的危險因素,除了無法改變的生物遺傳因素之外,當一方配偶罹患糖尿病,另外一方罹患的風險也跟著增加,揭示社會環境因素會影響糖尿病的風險。而糖尿病高危群的生活型態介入,可導致持續的生活型態改變,並長期預防發展為糖尿病。國內對於糖尿病配偶之研究較為缺乏,故需進一步探討。本研究目的,在了解糖尿病配偶與非糖尿病配偶在健康生活型態與糖尿病風險預測指數之關係。本研究方法,採橫斷式研究設計,以某醫院內外科病房,糖尿病配偶與非糖尿病配偶共兩組為研究對象,共200位。收案時間為2018年9月到2019年6月,使用人口學資料、健康促進生活方式量表及糖尿病風險預測指數問卷進行測量。研究結果顯示,(1)糖尿病配偶較非糖尿病配偶有較高的糖尿病罹患風險等級。(2)糖尿病組配偶的平均身體質量指數較高。(3)女性配偶、年齡越高時、有糖尿病家族史、身體質量指數越高者,有較高的糖尿病罹患風險等級。因此建議政府強化糖尿病配偶的健康識能,積極推動醫療院所糖尿病高危險群健康促進計畫。最後建議將配偶疾病史納入糖尿病臨床評估和風險評估工具中,以提高其識別未確診病例和高危群中的效用。

The risk of diabetes, apart from genetic factors that cannot be helped, as studies indicate the spouses of patients with type 2 diabetes encounter a greater risk of type 2 diabetes. This suggests that the socio-environmental factors can significantly increase the risk of diabetes. The lifestyle interventions to high-risk group of diabetes can lead to continuous changes in the lifestyle and prevent development of diabetes. There are only a few domestic studies and researches on diabetic spouses, so further research is desirable. The purpose of this study is to explore the relationship between spousal diabetes and non-spousal diabetes in predicting the risk of diabetes and promote healthy lifestyle. In this study, a cross-sectional survey was conducted with a total of 200 cases from both spousal diabetes and non-spousal diabetes recruited from the Medical and surgical ward of a hospital in northern Taiwan. Data were collected from September 2018 to June 2019. A set of questionnaires included areas of demographic characteristics, health promoting lifestyle profile, and Diabetes Risk Score. The results shown: (1) spousal diabetes had a higher risk of developing diabetes than non-spousal diabetes. (2) The average BMI index in spouses of patients with diabetes was higher. (3) Female spouse, greater age, family history of diabetes, and higher BMI index are factors contributing to higher risk of diabetes. Therefore, it is recommended that the government strengthen the health awareness of spousal diabetes and actively encourage the health promotion plan to high-risk groups of diabetes in medical institutions. Finally, it is proposed to incorporate the history of spouse's disease into the diabetes clinical assessment and risk assessment tool to improve the feasibility in identifying undiagnosed cases and high-risk groups.
