目前長期照護機構的各項評鑑、督考及輔導都強調感染管制，顯現感染管制在長期照護服務的重要性。本文以Donabedian的評估模式提供某護理之家感染管制之建構過程，包括：（一）結構面：1.有經認證之感染管制師以推動相關感染管制作業。2.制定護理之家感染管制作業規範、住民入住及篩檢評估作業、院內感染監測及通報系統並據以執行。3.設有充足的洗手設備以提升人員手部衛生之遵從性。（二）過程面：舉辦感染管制教育訓練及新興傳染病模擬及演習，並定期查核感染管制政策的執行情形。（三）結果面：訂定在結構面、過程面的監控標準，以評估其效果，進而檢討改善之。目前長期照護機構感染管制議題日益重視，期能藉由此經驗的分享，提供長期照護實務工作者參考，以促進長期照護機構在感染管制的品質。

Accreditation, supervision and counseling of the long-term care facilities address infection control, which underscores the importance of infection control in long-term care. This article demonstrates an experience in establishing an infection control surveillance process in a nursing home using Donabedian's structure-process-outcome model. 1. Structure: (1) Recruit certified infection control professionals to promote infection control operations. (2) Establish and execute operational manuals for infection control, pre-admission screening and admission assessment, as well as nosocomial infection surveillance and reporting system. (3) Provide sufficient sanitizing facilities to improve hand hygiene compliance. 2. Process: (1) Conduct infection control educational training programs, simulation exercises and drills for emerging infectious disease. (2) Inspect policy execution regularly. 3. Outcome: Formulate criteria for the structure and process for performance evaluation and improvement. Currently, infection control becomes more important in long-term care facilities. This article provided an experience of establishment of infection control surveillance process to promote the quality of infection control and performance in long-term care facilities.
