隨著人口快速的老化，如何減少醫院住院天數，且同時維持高品質的照護，成為醫療體系必須面臨的挑戰。因此，行政院衛生署開始試辦一系列包括亞急性與中期照護的計畫，並於2009年針對護理之家，推出「獨立型護理之家功能拓展計畫－試辦中期照護創新服務」實驗性計畫，期望未來護理之家能夠擔任急性醫療後期照護功能，以達到急性照護醫院及長期照護間無縫接軌的照護目的。然而，目前文獻對於亞急性／急性後期／中期照護術語的使用仍不一致，極易造成困惑，且有關國內護理之家是否具備亞急照護能力的文獻也較缺乏，故本文將就亞急性／急性後期／中期照護相關定義與發展予以說明，並針對護理之家照護現況與未來承接亞急性照護所面臨的挑戰，做進一步分析及提出建言。

Along with population aging, how to decrease the days of hospitalization and also to maintain the high quality of care for patients becomes the challenge for healthcare service system. Therefore, the Ministry of National Health of R.O.C. initiated a series of care program, which included the sub-acute and intermediate care. In 2009, the Ministry started an experimental program called as ”an intermediate care experimental program - expanding the independent function of nursing homes” and expected the nursing home will play the role of taking care sub-acute patients in order to achieve the goal of seamless care service. However, to date, the use of a variety of terms such as sub-acute care, post-acute and intermediate care is still inconsistent among literature; otherwise, there have been few studies which have examined whether the ability of nursing home in Taiwan could take the responsibility to take care sub-acute patients or not. The purpose of this article is to clarify the definition and the development of those terms including sub-acute, post-acute and intermediate care. In addition, the challenge for nursing homes to become the facilities for taking care those post acute patients would be analyzed and discussed in this paper.
