口腔衛生不良使口腔中積蓄大量細菌,衍生齲齒、牙周病與咀嚼障礙;衰弱的高齡長照者易因伴隨的系統性疾病而常發生吞嚥障礙。口腔中細菌隨著嗆咳、吸入物進入肺部,易罹患吸入性肺炎,嚴重影響全身健康與生活品質,更是臺灣高齡者第三大重要死因,不可不慎。然目前口腔照護尚未納入在長照2.0版本17項目中,但因臺灣高齡化指數急速上升,失能失智問題日趨嚴重,民眾對口腔照護相關知識及技巧認知的需求與日並增。如何能在高齡者失智失能以前就先做好口腔保健相關認知與技巧,積極面可藉由口腔刺激來增加他們的活力與減緩失智失能的速度,消極面可減少或減緩失智失能後對整體健康的衝擊;或對全身衰弱的高齡者能做好家屬及照護者的口腔照護訓練,使能減少口腔疾病的急速惡化導致整體健康更大的衝擊,減少吸入性肺炎的發生,最後能增進他們的健康及生活品質則是我們醫事人員的共同責任。 |
英文摘要 |
Poor oral hygiene may accumulate massive bacteria and then induce dental caries, periodontitis and mastication inability. Aspiration pneumonia is highly related with the massive bacteria in oral cavity invaded into lung tissue when dysphagia, aspiration attacked in frail elderly with systemic diseases. Aspiration pneumonia not only severely impacts general health and life of quality, but also ranks as 3rd position of cause of death in Taiwan. We should take care of oral care for frail elderly. The oral care for long-term care residents is not included in the 17 items of long-term care, version 2.0. Since the elderly index has recently been rising rapidly in Taiwan, the issues concerning people with disabilities and those with dementia have also been increasing dramatically. The general public has shown increased attention to health awareness, quality of life and oral health so, their demands for knowledge and skills of how to take care of their teeth and oral health is increasing simultaneously. So, how to educate the general public to improve their oral health related knowledge and skills before the age-related onset of disabilities and dementia is very important. Proper oral care stimulates vitality to decrease the tendency to disability and dementia. Moreover, in the event of age-related onset of disabilities and dementia, it will reduce the impact of oral disease to the general health. In addition, oral care education and training to the parents or caregivers of those with disabilities and dementia at an early stage will decrease the impact of oral diseases to the general health. This also applies to those suffering with aspiration pneumonia. Our ultimate goal and the medical professional's common responsibility is to improve the quality of life of those with disabilities and dementia. |
