本研究利用國家衛生研究院發表之全民健康保險研究資料庫,依據性別、年齡探討農民癌症及主要癌症部位的發生率,並採用Kaplan-Meier方法估計癌症發生後3年及5年的存活率,探討農民癌症風險。研究結果發現,農民癌症風險與非農民存在差異。男性農民除60歲至69歲之間的癌症發生率微幅低於男性非農民外,其餘年齡組中,男性農民癌症發生率相較非農民為高。對女性農民而言,40歲以後癌症發生率明顯低於女性非農民,在癌症好發之45歲至75歲間,差異比率可達12%以上。基於農民癌症風險與非農民之差異,商業癌症保險之發展與風險管理策略除了性別、年齡風險因子外,日後可研究發展滿足被保險人職業風險特性之保險保障需求。

英文摘要

This study examined the incidence of cancer and major cancer sites in farmers in Taiwan by sex and age using data from the National Health Insurance Research Database. The findings suggested differences in cancer risk between farmers and nonfarmers. First, the cancer incidence rate was slightly lower in male farmers aged 60-69 years than in male nonfarmers; however, male farmers in other age brackets were more likely to contract cancer than male nonfarmers were. Second, the cancer incidence rate was significantly lower in female farmers than female nonfarmers after the age of 40, with a difference of ≥ 12 % in cancer incidence rate between 45-and 75-year-olds. Given the differences in cancer risk between farmers and nonfarmers, not only should the development and risk management of commercial cancer insurance schemes account for risk factors such as sex and age, but further research should be conducted into how insurance needs can be met. according to the occupational risks involved.
