本篇為照護一位學齡前期兒童罹患肺炎的照護經驗,護理期間為101年11月12日至101年11月17日。筆者以生理、心理、社會、靈性四層面為評估架構,運用治療性遊戲、身體評估、非結構性的會談、觀察等活動收集資料。主要護理問題為體溫過高、呼吸道清除功能失效及恐懼。個案因疾病導致體溫過高和呼吸道清除功能失效,運用遊戲協助了解體溫過高的原因而配合治療。以繪畫、說故事、吹風車比賽等方式,引導個案學會深呼吸及咳嗽,接受胸腔物理治療。對於陌生環境和醫療治療感到恐懼,運用說故事方式引導個案表達出內心感受,並透過遊戲過程降低對於住院與治療的恐懼。提供照護資訊使家屬具備正確的照護技能,達成以家庭為中心完整性照護。 |
英文摘要 |
This article described a nursing experience of a 5-year-old child with mycoplasma pneumonia. The nursing period was from November 12, 2012 to November 17, 2012. The author used physical examination, unstructured interview, and observation to collect data and assess the physical, psychological, and spiritual needs of the patient. The results of nursing diagnosis were hyperthermia, ineffective airway clearance, and fear. The nursing strategies included maintaining body temperature within normal range, keeping airway clear, and reducing hospitalization fear. The patient showed physical improvement and reported less scared after therapy play and nursing strategies. This article shared a personal experience with nurses who will care for similar case in the future, with expectation of offering therapy play and nursing intervention with quality and family center care. |
