完整而完善之環境介紹可降低病人及家屬對陌生環境的焦慮、提升護理服務品質亦可降低意外事件之發生率,而一般傳統口頭介紹方式,常因護理人員的不同經驗,內容有所差異甚至容易有疏漏之處。於某外科病房針對新病人進行問卷調查發現,環境介紹之完整率僅27.5%。為能提升病人環境介紹之完整性,本專案提出之改善方案:拍攝環境介紹數位光碟於新病人入院及轉入時播放、制定環境介紹標準作業流程、環境中張貼圖片式標示牌,結果顯示:護理人員做環境介紹之完整率由27.5%提升至94.4%;病人因不了解病房環境而跌倒者,由61.5%降為0%;95%病人表示藉由環境介紹可降低住院焦慮;病人詢問環境相關問題累計頻次減少75%,顯示本專案能有效提升新病人住院環境介紹完整性,應可供其他相關單位參考。

英文摘要

A complete and comprehensive environmental introduction can reduce the anxiety of patients and their families against a strange environment. Enhancing the quality of nursing care can avoid accidents from happening. However, a traditional oral presentation, due to different experiences of nursing, often varies or even omits some important content easily. According to a survey interviewed by a surgical ward to new patients, the completion of the environmental introduction was only 27.5%. In order to provide a more complete environmental introduction to the patients, our project proposed an improvement plan as follows. First, we filmed the environment introduction into a digital disc, and played it when new patients were admitted and transferred. Second, we set up standard operating procedures, environment of the environmental introduction. Third, we posted pictures-style nameplates at hospital surroundings. The results showed that the completion of the environmental climbed from 27.5 % up to 94.4%; the number of patients who did not understand the ward environments and fall was cut from 61.5% down to 0%; 95% of patients said that the descriptions of the environment could reduce hospitalizations by anxiety. The cumulative number of times of the patients asked environment-related questions dropped down to 75%. These statics indicate that the projects could effectively enhance the introduction of new environment, and it should be available to other related organizations.
