運用行為修正法於一位慢性精神分裂症個案之護理經驗

本文乃筆者運用行為修正法於一位慢性精神分裂症患者之護理經驗，筆者透過整體性護理評估，發現個案因疾病慢性化後，雖可與殘餘的思考、知覺障礙共存，但因退化而影響自我照顧能力，導致挑食致體重僅25Kg，隨地解尿、吐口水致人際關係不良，護理期間為96年2月26日至96年11月15日，其主要的健康問題為：自我照顧能力缺失－進食方面、自我照顧能力缺失－清潔衛生方面。由於行為修正法的正性增強原則適切運用，及家屬與醫療團隊共同合作，使個案之不適當行為，以為人所接受的方式表現，亦減少不適當行為出現次數，用其所喜歡的食物為獎勵，進而改善其體重不足之問題。結果顯示：有效運用行為修正法，可改善慢性精神分裂症個案之自我照顧能力，提升生活品質。

This article is the nursing experience of the author using Behavior Modification to treat a Chronic Schizophrenic patient. According to comprehensive nursing assessment, the author found that the schizophrenic patient might have the problem with the coexistence of disorganized thinking and sensory disturbance. Degeneration would affect her self-attendance ability and cause her to become picky about food; therefore, her body weight reduced to 25 Kg. In addition, she also had bad interpersonal relationships with others because of her inappropriate behaviors of urinating and spitting randomly. The nursing period was from 02/26/2007 to 11/15/2007. The main health problem on losing self-attendance ability was in the aspect of dieting and hygiene. With conducting positive reinforcement techniques of Behavior Modification properly and cooperating with her family and the medical team, the patient was able to change her improper behaviors to acceptable ones. Besides, the underweight problem would solve as well by using her favorite food as a reward. This result shows that by applying the principles of behavior modification effectively, the patient with Chronic Schizophrenia could improve her self-attendance ability and promote her quality of life.
