本文描述一位長年安置於機構中的思覺失調症病人，因機構傳染問題導致生活環境異動、焦慮情緒加劇。照護時間為2018年1月23日至2月23日，進行精神科五大層面整體性評估收集資料，護理問題為：一、身體清潔／與疾病（精神症狀）、缺乏動機導致自我照顧能力功能退化有關。二、焦慮／與新環境及對於未來安置不確定性有關。三、知覺感受變異（幻聽）／與疾病影響聽覺產生改變有關。筆者運用復元理論正向觀點提升自信心；參與決策能力提升賦權，並結合另類療法，如音樂治療轉移精神症狀干擾，使其放鬆、焦慮情緒降低；園藝治療培養專注力與懷舊治療提昇認知能力，回憶過往美好記憶，找到人生價值、促進生活滿意與適應。

This article describes a schizophrenic patient who has been placed in an institution for many years. Due to institutional infections, the living environment has changed and anxiety has increased. The care time was from January 23 to February 23, 2018. Data were collected through observations, interviews, etc., and the five aspects of the overall nursing assessment were used to establish the nursing problems: Firstly, the body was clean related to the disease (psychotic symptoms), but lack of motivation had led to the deterioration of self-care ability; secondly, there was anxiety related to the new environment and uncertainty about future resettlement; and finally, the perception of variability (auditory auditory) was related to the impact of disease on hearing changes. The author used the recovery theory to enhance self-confidence from a positive point of view; encouraged the patient to participate in decision-making to enhance empowerment, and combined alternative therapies such as music therapy to transfer mental symptoms, to relax, reduce anxiety; and finally introduced horticultural treatment training focus and nostalgic treatment to enhance cognitive ability. In recalling the good memories of the past, the patient developed abilities in finding the value of life, thereby promoting life satisfaction and adaptation.
