背景:燒燙傷病人與身體改變共存時間長,了解其生命經驗才能提供適切護理照顧。目的:運用質性分析了解一位燒燙傷病人之經驗歷程。方法:自2020年7月14日至2021年4月15日照護期間進行三次訪談,傾聽個案敘說其燒燙傷經驗。訪談內容轉錄逐字文本進行質性分析。結果:個案歷經急性創傷、身心調整與回歸生活之燒燙傷歷程。個案燒燙傷雖非外顯部位,仍在意外觀與觸感,並積極照顧身體。燒燙傷皮膚對熱敏感,引發情緒與社會照顧需求。個案回歸職場避免再度接觸造成燒燙傷事件原因,與創傷後壓力症候群相符。結論:燒燙傷個案護理重點包含疼痛、資源需求、身體症狀與疤痕、情緒,以及創傷後壓力症候群的評估與照顧。陪伴其度過治療與復原歷程,應為全人照顧方向。

英文摘要

Background: Patients suffered from burn injury need long period to compromise and tolerant with the physical changes. Only when the life experience of the patients is understood can appropriate nursing care be provided to them. Purposes: Using qualitative analysis to understand the experience of a patient suffering from major burn injury. Method: This study provided three interviews from July 14, 2020 to April 15, 2021 during the care period of the cases. The author listened and recorded the burn experience of the cases and converted the interview content into transcripts and proceeded the qualitative analysis. Results: The cases experienced the burn processes: acute trauma; physical and psychological adjustment; and returning to normal life. Although the remain scars were invisible under clothing, the cases still cared about the body's appearance and sense of touch, so they put more attention to care their skin. After experiencing the burn injury, the patient's skin was sensitive to heat, which would trigger the cases' emotional and social care needs. When returning to the workplace, the case would pay attention to avoid re-exposure to the cause of the burn event, which was consistent with post-traumatic stress disorder. Conclusion: The focuses of care for the patient with burn injury include the assessment and care of pain, resource needs, physical symptoms and scars, emotions, and post-traumatic stress disorder. During the caring process, it is important to continuously accompany patients to go through the processes of treatment and recovery, which is the holistic care direction for patients with burn injuries.
