本文照顧一位喝鹽酸企圖自殺之重鬱症病患行食道造口及空腸造瘻留置後轉住精神科急性病房照護經驗。照護期間2020年5月14日至6月16日，個案首次精神科住院，運用觀察、會談及閱讀病歷進行整體性評估，確立主要健康問題有潛在危險性自殺、失眠及身體心像紊亂。護理過程利用Swanson關懷理論，運用接納、尊重、陪伴及關懷，建立治療性人際關係，擬定個別性護理計畫，使其願意了解內在感受和自我認同，訓練正向思考、學習照護技能和壓力因應技巧，提升問題解決能力、發展有效適應模式、協助資源連結和後續安置，建立有效支持系統，進而維持健康狀態和降低自殺風險，故分享此護理經驗，提供護理人員對Swanson理論之護理運用，並提升重鬱症合併自殺企圖之照護品質。

This article describes a nursing experience of caring for a patient with severe depression who drank hydrochloric acid and attempted suicide with esophagostomy and jejunostomy indwelling. During the nursing period from May 14th to June 16th, 2020, the case was hospitalized for the first time in the psychiatric department. An overall evaluation was carried out using observation, interviews and reading medical records, and it was determined that the main health problems were potentially dangerous suicide, insomnia and body image disorders. The nursing process uses Swanson's caring theory, uses of acceptance, respect, companionship and caring to establish therapeutic interpersonal relationships, and formulate individual nursing plans, so that they are willing to understand inner feelings, re-identify yourself, train positive thinking, learn care skills and strengthen stress coping skills, improve problem-solving ability, develop effective adaptation models, assist in resource connection and follow-up placement, establish an effective support system, and then maintain health status and reduced suicide risk, so share this nursing experience, to provide nursing staff with the nursing application of Swanson's caring theory, and improve the quality of care for major depression patients with suicide attempts.
