此個案報告運用Peplau人際關係理論,照顧一位適應疾患的青少年之護理經驗。筆者透過五大層面護理評估及運用Peplau人際關係理論進行個案與護理人員之行為分析,發現個案有自我殘害、社交互動障礙與個人因應能力失調等護理問題,以及筆者面臨個案反覆自傷行為產生挫折與壓力,以Peplau人際關係理論的介紹期、認同期、探索期及解決期,運用六大角色功能如替代者、領導者、資源提供者、教導者和諮商者,漸進式與個案建立關係進而取得信任並共同討論。結果顯示,運用Peplau人際關係理論照顧一位適應障礙青少年的個案,可以導正個案的偏差觀念及問題行為,建立正向的因應壓力方法,而筆者從照護此個案的過程中也獲得學習與成長。

英文摘要

This case study reported the application of Peplau's theory of interpersonal relation model in the nursing of a teenager patient with adjustment disorder. Through the five-dimension nursing evaluation and the application of Peplau's theory in the analysis of the patient and the nurse's behavior, it was found that the patient suffered from self-injury, social interaction impairment and adjustment disorder. The nurse also faced the problem of frustration and pressure due to the patient's repetitive self-injury behavior.The nurse used the four phases of Peplau's theory: orientation, identification, exploitation and resolution, and the six roles, such as surrogate, leader, resource person, teacher and counselor, to earn trust and engaged in a mutual discussion with the patient. During the nursing process, the nurse constantly engaged in self-retrospection.The results showed that the adaptation of Peplau's theory to nurse a teenager with adjustment disorder can correct the patient's misconception and problematic behaviors and construct positive methods of coping with pressure. Moreover, the nurse has also learned and matured during the caring process.
