運用行為治療於一位重複住院自閉症青少年的護理經驗

本文探討一位自閉症青少年，因反覆出現暴力及偏差社交行為而重覆住院的護理經驗。照護期間為2012年4月2日至2012年5月9日。藉由觀察、會談及查閱病歷等方式收集資料，運用身體、情緒、智能、社會、靈性五大層面護理評估，確立個案有現存性暴力、家庭因應能力失調、睡眠型態紊亂、社交技巧缺失、自我照顧能力缺失及營養狀況改變等健康問題。經醫療團隊與個案、家屬取得共識，依個案個別性擬定行為治療計劃並操作，個案不當行為明顯獲得改善，家屬從中學習行為治療之技巧延續於出院居家生活中，增加對個案症狀的因應能力，個案順利返回學校生活。顯示行為治療對自閉症疾患不當行為的矯正有顯著的成效，期望藉此照護經驗供臨床護理同仁參考。

This article was to explore nursing experience of applying behavior therapy to an autism adolescent who was re-hospitalized due to violent attacksdue to and deviant social behavior. The period of nursing care is from April 2nd to May 9th 2012. Data was collected by observation, interview, and review of medical records. The nursing problems we identified included violence, ineffective family coping, sleep pattern disturbance, impaired social interaction, self-care deficit and altered nutrition. Medical team, patient and family discussed together and devised treatment plan to deliver behavioral therapy. We found less deviant social behavior was noted, and family could apply the skills based on behavioral therapy to cope with the challenges after discharge. The patient finally returned to school. It was shown that behavioral therapy could improve autistic disorders.
