本篇報告描述一位氣喘兒童不遵從用藥,在二年內八次因氣喘急性發作住院治療的護理經驗,護理期間自2013年10月16至10月21日,筆者運用Gordon十一項功能性健康型態評估,藉由觀察、會談、身體評估及傾聽等技巧收集資料,確定個案主要健康問題為:低效性呼吸型態、服藥不遵從與知識缺失。筆者依個案年齡及認知發展程度,運用治療性遊戲加強病童執行肺部擴張運動,以衛教圖片教導病人認識氣喘用藥,共同設計個人化的服藥卡增加用藥的遵從度,示範與回覆示教正確吸藥技巧。最終,個案在出院前能說出氣喘藥物治療的重要性、正確操作乾粉吸入器、養成正確服藥習慣並上氣喘諮詢網搜尋相關資訊。藉由此個案的護理經驗分享,作為護理人員照顧學齡期氣喘兒童服藥不遵從之參考。 |
英文摘要 |
This study described the nursing experience of a schoolage child who fails to comply the prescribed treatment regimens. The nursing period lasted from October 16 to 21, 2013. We collected data through interview, listening, observation and physical assessment. The Gordon's 11 functional health assessment was used to identify major health issues, and the following three were identified: "ineffective breathing pattern", "deficient knowledge", and "non-compliance". Based on the child's developmental stage, we applied multiple therapeutic activities to improve the patient's lung function through breathing exercises, increase asthma knowledge via health education pictures and personalized medication log, and increase medication compliance by demonstrating dry powder inhaler techniques. Ultimately, the child had learned the medical importance to asthma control, adhered to medication, correctly operated dry power inhaler and actively searched asthma related information on the internet. We wish that this nursing experience could be used as a reference by other professionals when encountering similar cases. |
