本報告在描述照護一位33歲男性年輕型小腦中風個案的護理經驗。筆者於2013年9月10日至10月1日照護期間，運用Gordon 11項健康功能型態，以身體評估、觀察、會談、傾聽及溝通等方式收集資料，確立個案主要的健康問題為：「感覺知覺改變」、「身體心像紊亂」、「無力感」等。於護理過程中給予關懷並與復健師討論且修正復健計畫，協助把握復健黃金期並順利回歸工作。個案僅33歲即中風，帶給自己及家屬極大震撼、擔憂與焦慮。由於小腦中風後引發肢體活動平衡功能不佳，嚴重影響個案步態活動及生活品質。照護期間藉由個別化的護理活動，提供正面思考的機會及心理支持，增進個案自我控制感，協助於小腦中風後的身心壓力調適。臨床上基於人力與時間限制下，建議護理人員照顧類似個案時可考慮跨領域團隊照護，以提供患者更周全與完善的照護。

This care report described the nursing experience of a 33-year-old male patient with cerebellar stroke. During the nursing period from Sept. 10^(th) to Oct. 1^(st), 2013, the author organized the data collected from physical evaluation, observation, interviews, listening, and communications according to Gordon's 11 functional health patterns, and established the following health issues: altered sensory perception, body image disturbance, and powerlessness. For the patient to return to work after stroke, the author tended to the patient while discussing and adjusting his rehabilitation plan with his physical therapist. Since the patient was only 33 years old when he had the stroke, both the patient and family felt immense shock, grief, and anxiety. Cerebellar stroke often affect movement and balance, which could severely impact the patient's gait activities and life quality. Through individualized nursing care, the author facilitated positive thinking and provided psychological support to the patient to enhance a sense of self-control, which in turn benefited the patient's physiological and psychological adaptation to stroke. Clinically, under the constraints of time and staff, we recommend the nursing staff to consider interdisciplinary team care when encountering patients with similar conditions, to ensure the provision of comprehensive nursing.
