護理人員準時下班是目前炙熱的話題,護理工作常因人力不足、工作時間過長,而有身心俱疲、工作滿意度下降、流動率高進而影響病人安全與照護品質之擔憂。本專案自2012年7月10日至2013年02月28日經專案小組深入探討,發現單位有「點班、交班時間過長」、「白班照護量大」、「接新病人花費時間長」、「醫療儀器保養時間耗時」等問題。專案小組經現況分析及文獻查證,提出改善方案如1.修訂交班規範,2.修訂職務說明書,3.修訂醫療儀器管理,4.制訂接新病人工作職責,5.宣導點班規範等;因此單位準時下班率由23.8%提升至90.6%,下班平均延遲時間則由49分鐘降低至15分鐘。故經此專案進行確實提升護理人員準時下班率,期望能平行推廣於服務機構之各護理單位,以嘉惠更多臨床護理人員。

Nursing staff leaving work on time has been a topic of conversation. Nursing work is often faced with staff shortage and prolonged hours, which then lead to physical and mental fatigue, decreasing job satisfaction, increasing turnover rate, and in turn raise concerns for nursing quality and patient safety. The project was launched in the pediatric intensive care unit on July 10, 2012, and ended on Febrauary 28, 2013. An in-depth investigation conducted by the project team revealed that "too much time spent during inventory and shift change", "excessive dayshift workload", "too much time spent receiving new patients", "medical equipment maintenance too time consuming" and etc. After situation analysis and literary review, the project team proposed the following solutions: revised regulations on shift change; revised job description; revised medical equipment management; revised job description on receiving new patient; and advocate inventory regulations. The rate of nursing staff leaving on time increased from 23.8% to 90.6% after the solutions were implemented, and the averaged delayed time reduced from 49 min. to 15 min. The project was effective in increasing the rate of nursing staff leaving work on time. We would like to advocate and implement the project in all nursing units of the medical institution in hope to benefit more clinical nursing staff.
