A Post-Operative ICU Nursing Experience of a Middle-Aged Tongue Cancer Patient

本文描述一位46歲中年罹患舌癌術後於加護病房護理經驗。照護期間2015年5月13日至2015年5月18日，筆者運用Gordon十一項健康功能型態評估個案，藉由直接照護、身體評估、觀察進行資料收集、記錄，確立個案之護理健康問題。歸納分析個案護理問題有急性疼痛、組織完整性受損、身體心像改變。於加護病房護理過程中，運用重症疼痛觀察評估工具，紀錄疼痛並緩解不適；術後密切監測皮瓣血流，減少移植失敗；面對身體外觀改變，主動關懷建立信賴感，並以同理心及正向鼓勵態度引導個案坦然面對疾病，增進自信，恢復人際互動關係，緩解個案對治療所帶來的不安與衝擊。

This paper described a post-operative ICU nursing experience of a middle-aged male tongue cancer patient. The nursing period lasted from May 13^(th) to 18^(th), 2015. With Gordon's functional health patterns as framework for assessment, the author collected data via direct care, physical assessment, and observation, and confirmed and documented the patient's health issues as acute pain, impaired tissue integrity. and body image change. During the nursing period in ICU, the author used the critical-care pain observation tool to document the patient's pain and relief discomfort; observed flap blood flow to avoid graft failure; building trust with the patient through active care to address the body image change, and facilitate positive attitude, self-confidence, and the restoration of social interaction with empathy and encouragement, relieving the uncertainty and impact the patient had experienced as the result of the treatment.
