運用治療性遊戲於一位燙傷學齡前期兒童之護理經驗

本文探討一位6歲兒童因翻倒熱湯而住院之護理經驗，護理期間自2017年8月27日至2017年9月13日，運用Gordon十一項健康功能性型態進行評估，確認健康問題有急性疼痛、皮膚完整性受損及恐懼。筆者運用治療性遊戲與案童建立良好護病關係，藉由疼痛緩解技巧減輕個案換藥時導致的傷口疼痛，提供燙傷傷口癒合相關照護知識及預防感染注意事項，透過主動關懷、陪伴玩耍及心理支持，鼓勵案童表達住院治療期間遭受的恐懼與不安感，適時評估生理及心理需求，給予適當護理措施及衛教指導，提升住院期間之護理照護品質。藉此經驗分享做為日後臨床相關護理個案照顧之參考。

This case report describes the nursing experience of a pre-school child hospitalized with burns resulting from overturning hot soup. The nursing period from 27th August 2017 to 13th September 2017. The health problems identified by Gordon's 11-item functional health assessment were acute pain, impaired tissue integrity, and fear. To build a better relationship with the patient therapeutic play was used. Pain relief techniques were used to reduce wound pain caused by changing dressings, to provide knowledge of scald-related wound healing, and to prevent infection. An aggressive rehabilitation therapy was also given. Companionship and psychological support were an important part of this intervention used to encourage the child to express their fearful and anxious experiences during hospitalization. With timely assessment of physiological and psychological needs, appropriate child-centered nursing measures and health education guidance to improve the quality of nursing care during hospitalization, we expect sharing this experience could be of value to clinical nursing with similar patients.
