本文協助一位腦中風病人之照護經驗，照護期間2018年3月19日至2018年4月16日，筆者以觀察、會談、身體評估及運用Gordon十一項功能性健康型態評估，歸納個案有「身體活動功能障礙」、「吞嚥障礙」及「照顧者角色緊張」等問題。經主動關懷、陪伴及主要照顧者建立護病關係，並透過傾聽及鼓勵主要照顧者說出照護困難點，緩解緊張情緒，提升主要照顧者的照護能力，同時，以跨團隊整合照護模式，協助個案克服因疾病導致的身體失能及吞嚥困難，提供規律復健運動治療，避免肢體攣縮及促進肢體關節的活動度；教導吞嚥訓練，免於吸入性肺炎發生；提升個案及主要照顧者生活品質。

This article described the nursing experiences of assisting a stroke patient and her caregiver from March 19th to April 16th 2018. The authors utilized Gordon's 11 functional health scales for collecting information, and to analyze and sort out the health problems based on the patient's physical assessment, close observation and interview with the patient and her significant caregiver. The major problems were: ＂Mobility impaired-physical＂, ＂Dysphagia＂ and ＂Caregiver role strain＂. The authors spent time with the patient and her caregiver together to better establish a good client-nurse relationship. Then, active care, concern, and empathic listening was provided to relieve the caregiver's stress and to encourage him to describe difficulties and any problems to caring for the patient. Relevant nursing activities were provided to offer the caregiver know-how for improving his ability to take care the patient. Interdisciplinary care models were also integrated to help the patient to overcome physical disability and difficulty swallowing with a regular rehabilitation program including swallow training, prevention of limb contractures and occurrence of aspiration pneumonia, with the aim of improving quality of life for the patient and her caregiver.
