本專案旨在運用病人參與策略，提升住院病人防跌措施的完整性，經現況調查確立問題為：病人防跌措施完整性低，僅37.9％。分析原因有：「覺得不會跌倒」、「忘記衛教內容」、「沒有拿到衛教單張」、「衛教單文字看不懂」。專案施行措施包含：設計參與式衛教圖卡、製作防跌衛教手冊、規劃衛教教材擺放位置，以及規劃QR code衛教影片專區。專案施行後，病人防跌措施完整性從37.9％提升至85.2％。顯示本專案的推動有效提升住院病人參與防跌倒措施的完整性，降低住院病人跌倒發生率，促進病人安全。

In our recent investigation into fall prevention, the integrity of patient fall prevention measures was found to be low at only 37.9%. Analysis indicated patients' reasons given for this result are: ＂I don't think I will fall＂, ＂Forgot the content of the health education＂, ＂I didn't get the health education leaflet＂, and ＂I can't understand the text of the health education leaflet.＂ This project aims to use patient participation strategies to improve the integrity of inpatient fall prevention measures. The implementation measures of the project include: designing participatory health education chart cards; making anti-fall health education manuals; planning the placement of health education textbooks; and planning a QR code health education video area. After the implementation of the project the completeness of patient fall prevention measures increased from 37.9% to 85.2%. This outcome shows that the promotion of this project effectively improves the integrity of inpatients participating in anti-fall measures, reduces the incidence of inpatient falls, and promotes patient safety.
