本文探討一位因失智症前期併糖尿病末期腎病變患者的照護經驗，護理期間為2019年03月19日至03月25日，筆者運用老人周全性評估，經實際照護及會談收集資料，確立個案有體液容積過量、無效性健康維護能力、焦慮等護理問題。護理期間提供個別性措施與指導，增進病家的疾病照護與因應能力，減輕焦慮，及透過跨團隊整合照護，提供政府輔助資源管道，及返家後持續性照護，有效的減少併發症發生，並達到疾病控制與預防目的，減低家庭負擔。失智症已是國人嚴重的慢性病之一，讓失智症症狀與其他共病能獲得妥善的醫療與社會、心理照護，及失能支持服務，也是現今重要的公共衛生議題，希望藉此照護經驗，做為日後照護此類個案之參考。

This article discusses the holistic care experience of a patient who has pre-dementia with complications of diabetic nephropathy. The care period was from 19 March to 25 March in 2019. Relevant patient data was collected through physical assessment and interviews. The patient was comfirmed with nursing problems including, excess body fluid volume, ineffective health-maintenance ability, and anxiety. During the care period we provided an indivisual care plan for the patient and his family in order to reduce the patient's anxiety. Integral nursing care was provided that included government resources and offered continuous nursing care aimed at disease control and prevention and reducing complications of the disease efficiently while the patient went back to society. Dementia has become a serious chronic disease and an important public health issue for the country, and so demands proper medical treatment, social and psychological care, and disability support services. It is hoped that this nursing care experience can establish a further reference for nurses caring for similar patients.
