Background: Anterior knee pain is one of the most common orthopedic knee disorders. The pain could be caused by an excessive lateral patellar displacement that leads to strain on the retro-patellar subchondral bone. Methods: In this prospective case study, the effectiveness of electro-acupuncture with direct current in three male adult patients with anterior knee pain was documented with magnetic resonance images. The patients were treated using nine acupuncture needles inserted into the iliotibial band with an electric current of 12 volts and 200 micro-amperes applied for 30 seconds; the treatment was repeated weekly 6 times. Results: After 6-weeks treatment, a comparison between pre- and post-treatment images reflected that the lateral patellar displacements were reduced, those biomechanical changes likely contributed to a decrease in numeric pain rating scale reported by the patients. Conclusion: The positive finding for the electro-acupuncture in the present study warranted further investigation.
