本專案目的為運用跨團隊模式降低血液腫瘤科病人跌倒發生率,藉由文獻查證與資料分析跌倒原因為病人雙下肢無力及預防跌倒認知不足、照顧者缺乏照顧技巧及預防跌倒認知不足、衛教工具不理想、浴廁高低台階及無扶手輔助等,經由團隊合作加強病人及照顧者對預防跌倒認知、進行照顧者預防跌倒照顧之技巧衛教、製作衛教圖示卡及預防跌倒海報、協同工務室修改浴廁及會診復健師安排抬臀運動,並請社工師提供相關資源等措施,結果顯示血液腫瘤科病人跌倒發生率由0.16%降至0.07%、2級以上傷害程度由4%降為0%、病人及照顧者預防跌倒認知由72%提升至95%、病人及照顧者對衛教工具滿意度由76%提升至100%、安排復健師執行抬臀運動指導,使病人肌力由3分提升至5分,且提升病人及照顧者住院滿意度。期盼將此專案的推動模式,提供給類似案例參考使病人安全無虞。

英文摘要

The purpose of the protocol was to apply the cross-team model to reducing the falling down rate in patients atthe oncological wards. Throughthe literature review and data analysis, the factors contributed to the patients' falling downs are: bilateral lower limbs weakness, lacks of knowledge on preventing the falling down events, inadequate care-giving skills as well the ignorance of preventing falling down, the unsatisfactory tools for health education, no armrest auxiliary while walking on the bathroom floor with high and low steps, etc. Through the team cooperation, we enhanced the cognition of the patients and caregivers about the prevention of falling down, through educating caregivers the skills to prevent falling down, made the illustrated pictures and posters about the prevention of falling down, remodeled the auxiliary facilities in the bathrooms with the Engineering Department, consulted the rehabilitation members to do the lift buttock exercise, and discussed with the social workers aboutthe associated resources, etc. The results shows the falling down rate in the patients atthe oncological ward decreased from 0.16% to 0.07%; the degree of injury above grade two diminished from 4% to zero; the patients and care-givers' cognition of preventing falling down elevated from 72% to 95%. In addition, we also arranged the rehabilitation colleagues to introduce the lift buttock exercise that could increase the patient's muscle power from point three to point five. Furthermore, we improved the inpatients and caregivers' hospitalized satisfaction, and also increased the collaboration of the medical team. We sincerely hope that the promotion of the protocol could share the experiences with patients and caregives at the oncological wards as a reference, and ensure patients' safety.
